Exam Excuse Request Form |

For the Fall 2025, this form will be available until Friday, 12/12/2025.
Use this form if you meet the following criteria:
- Your primary major is within the College of Arts and Sciences.
- You have two final exams scheduled at the same time, three exams which start within one 24 hour period (ie, 8am – 8am, noon-noon, etc.), or you have approved EOC/ARS accommodations impacting your overall testing time and know the details of your accommodations.
- You know which exam you wish to move. Per UNC policy for fall and spring semesters, if one of your courses is listed in the final exam schedule under the section for common hour exams, then the excuse must be used for that class.
- You have another conflict that requires a Dean's review (examples: religious observation, military obligation, medical emergency, etc.). You will need to upload appropriate documentation when completing the form.
Exam time references:
Degree-seeking students who need to request an exam excuse based on a medical or mental health issue prior to or on the day of the final exam must go through the process outlined by Campus Health(see "Illness During Final Exams"). Non-degree students should utilize this form and be prepared to upload documentation.
